In order to regulate the termination or withdrawal of permission to apply for the administrative work of cosmetics, will now go through the program and notify the relevant requirements are as follows:
First, the applicant withdraw the application termination or administrative license cosmetics, should log the State Food and Drug Administration Web site cosmetics administrative license online reporting system, and fill out the application form.
Second, the information submitted by the applicant to declare the administrative license, the State Food and Drug Administration accepted the executive agency (hereinafter referred to as the administrative appellate body) did not make the decision on admissibility, the original applicant may apply in writing to terminate the administrative appellate body report (see Annex 1 Application Form ), the executive agency shall accept the date of receipt of the application within five working days to make a decision whether to grant the termination. Termination of the grant, the date of termination within five working days, the administrative appellate body report all information should be returned to the applicant.
Third, the administrative licensing declarations submitted by the applicant, the administrative appellate body has accepted the technical review before the meeting, the original applicant may apply to the administrative appellate body a written application for the withdrawal of administrative licensing.
(A) withdraw the application for administrative license shall submit the following declarations 1:
1 to withdraw the administrative license cosmetic application form (see Annex 2);
(2) of the original notice of acceptance of the original administrative license;
3 agent declaration, shall be submitted to the administrative licensing has been filed to declare the responsible units in China, power of attorney a copy, and reporting responsibilities of the administrative licensing unit in China business license and official seal;
4 other help to withdraw application for administrative license information.
(B) the withdrawal of the license application for working hours. Disclosure of information submitted by the applicant to meet the requirements of the State Food and Drug Administration should be within 20 working days for withdrawal of the license application procedures.
Allowed to withdraw the application for administrative license, the date of withdrawal of approval within 15 working days, receiving institutions should report all data returned to the applicant.
(C) the applicant can log on the State Food and Drug Administration Web site cosmetics online reporting system of administrative licensing, check to withdraw application for approval of administrative licensing.
(D) of the State Food and Drug Administration review of administrative agencies to the application for leave to withdraw the relevant information should be timely filed.
